I've been using the nightly Synchronet builds for Windows (upgraded periodically), and for a while now, I've noticed that when I exit sbbsctrl, it shows the error "Failure writing initialization file: D:\BBS\SBBS\ctrl\sbbs.ini".

I don't think I have any file permission issues.. Any suggestions to what might cause that error?

I've checked the file's attributes, but neither the 'read only' nor 'hidden' are applied. I'm not sure what else might be causing the problem.

try moving it. close the bbs. move it back. open the bbs.
also when is the last time you ran chkdsk?

Windows has a lot more fine-grained access control than just 'read only' and 'hidden'. Check who the owner of the file is and what rights you've given to what Windows user groups.
